We make filing insurance claim forms easy by electronically filing them for patients with all companies with which we are participating providers. For the few others we provide a detailed itemized statement at the time of each visit. That form is accepted by every insurance company we know when accompanied by a claim form with the appropriate personal information completed by the patient. If extra copies are needed, we are happy to photocopy them for you at the time of the visit at no charge.

Occasionally a family loses one of those forms. Upon your request, we will provide you with an itemized statement of your account over the past year, signed by our staff certifying that it is a true statement of the services obtained from our office. These account summaries, which are accepted by insurance carriers, are run off in a batch every two weeks or so – unfortunately, therefore, they cannot be provided “on demand.”

If your insurance company wishes further documentation of a visit, a special report, etc., have them make a request to us in writing. That request must contain your signed authorization for release of information. These reports are provided in a timely fashion. Fees for physician and secretarial time producing them are billed to the insurance company directly.
