We always face a rush of physical exams and forms to be completed during the late spring and summer. These are required for camps, schools, and colleges, and must be completed before a student can participate. Please consider making your appointment for such exams now while our schedule still has many openings. Please allow 4 business days for completion of forms and completed forms can be picked up in the Essex office.  All forms, except for State approved Blue and Yellow Forms, will be completed for a $10 fee.  If forms are required and need to be a "rush" there is an additional $10 charge.
